Health records will be retained for a period of five (5) years from the patient's last visit. In the case of patients under twenty-one (21) years of age, the records will be kept until the minor reaches the age of twenty-two (22).
Once the established period has concluded, the facility will retain the records for an additional five (5) years. Afterward, they will be disposed of in accordance with the law. In cases where a patient has not received medical-hospital services for a period of ten (10) years and wishes to keep their health record, they may request it from the institution. Patients who are still receiving services and wish to retain the original health record will be provided with it as required by law. However, the patient must sign a document releasing the institution from the responsibility of safeguarding the health record and the integrity of the information contained therein.

To request a copy of a health record, the applicant must visit the Health Information Supply area of the Health Information Department and follow the following procedure:
Complete the Disclosure Authorization form specifying the information desired.
If the requester is not the patient, they must bring a signed authorization from the patient along with photo identification for both individuals.
This process incurs a service charge of $0.75 per photocopied page, in addition to $5.00 for certification.
The schedule for health information requests is Monday, Tuesday, and Thursday from 8:30 am to 1:00 pm; Wednesday from 8:30 am to 3:30 pm; and closed on Fridays.
The processing time for copy requests is 15 business days. Some requests might be fulfilled on the same day as requested.

Under the HIPAA Law, individuals have the right to request access, amendment, or corrections to their protected health information. These requests must be made directly to the Health Information Department, Health Information Supply area. Cases involving access, inspection, and amendments will be addressed by appointment, once the request for these purposes is received.
